Equitable access to health and aged care

Not for Profit, integratedliving™ Australia is working with a series of partners to trial an innovative model of care to give older Australians and those with chronic health conditions in regional, rural and remote communities improved choice and quality services for health and aged care.

The long-lived case of inequity was highlighted by the Royal Commission into Aged Care Quality and Safety, and prompted integratedliving™ to partner with Primary Health Networks in Queensland (including Gold Coast, and Central Queensland, Wide Bay & Sunshine Coast), and local health and hospital services.

Gold Coast and Central Queensland, Wide Bay & Sunshine Coast PHNs, are trialling the new model, called the Integrated Health Systems Demonstrator, which leverages digital technologies and multidisciplinary care to combine virtual and face-to-face care.

Clients will interact with their health care team via the Core by integratedliving™ mobile App, enabling communications via phone, chat, email and video as well as access to their scheduled services, care plans and other documents. This person-centred model of care will incorporate both traditional aged care services with clinical services through a mix of on-location (face to face) and virtual methods of health service delivery.

This is about providing continuity of care and easy to use wrap-around service with a single point of entry for people who really need it.

H/Advisors APA was pleased to work with integratedliving™ to take their solution to decision-makers at a Federal and State level, across PHNs and the health and aged care sectors.  Some early successes and the ability to demonstrate impact will likely see the model rolled out to more locations throughout regional, rural and remote locations into the future.
